If you think it was just a mistake, you need to contact the creditor and the credit bureau directly to follow their dispute process. It is generally easier to file a dispute online.
For United States
The three credit bureaus are:

- Equifax: https://www.equifax.com/personal/credit-report-services/credit-dispute/
For Canada
The two credit bureaus are:
